Identifying Purkinje Involvement in Ventricular Fibrillation Substrate

Editorial Commentary

This editorial commentary examines the critical role of the His-Purkinje system in ventricular fibrillation pathogenesis and the therapeutic implications of fascicular ablation for treating recurrent ventricular arrhythmias. The Purkinje network, despite representing only 2% of ventricular mass, plays a disproportionately important role in initiating and maintaining ventricular fibrillation.

The authors discuss current challenges in standardized identification of Purkinje involvement and propose future approaches to improve treatment outcomes for patients with structurally normal ventricular fibrillation.
